If you would like your order to be delivered directly to your organization (address and phone number listed above) and avoid shipping charges, list the name, address and phone no. of that organization under the "SHIPPING" section of the order form along with your e-mail address.  List the organization name again under the "COMPANY" line. If you would like your order shipped directly to you remember to list the organization name under "COMPANY" in the "SHIPPING" section in order for that organization to receive the 10% donation.

If delivering to the organization, click the box marked "Enter new billing information".  If having item shipped to you, click the box marked "Same as Shipping information".

List your name and billing address, phone and credit card information under the "BILLING" section of the order form.
